How to Apply for Access to Washington State HMIS

Each HMIS User must be training prior to accessing the HMIS.  Please contact the HMIS Coordinator to make arrangements. Once training is complete, the HMIS Coordinator creates a username and password for your access to HMIS.

• Agency Partner Agreement – Each agency must sign this and submit it to the Department of Commerce prior to participating in WA HMIS.

• Client Notice of Uses & Disclosures – Your agency is required to post this notice on the wall in a common area. 

• Client Informed Consent – Each client must sign this form to consent to or deny the collection of his or her personally identifying information.  Also available in Spanish.

• Client Revocation of HMIS Consent - If your client decides to revoke consent, they must sign this form.

• User Code of Ethics, Policy, and Responsibility Statements - Each WA HMIS user must complete this form prior to using HMIS.
